PHYSIOTHERAPY THAT COMES TO YOU

Who is home physiotherapy for?

Home physiotherapy can be particularly useful if travelling to a clinic is difficult, you have recently left hospital, have reduced mobility, are recovering from a stroke or neurological condition, have experienced falls, or simply prefer to be assessed in your own environment.

Your home is part of the assessment.

Being assessed at home allows me to see the things that actually affect your day-to-day mobility — such as stairs, furniture, transfers, walking routes and equipment — so rehabilitation can be more relevant to your everyday life.

YOUR HOME VISIT

WHOLE-PERSON SUPPORT

More Than Physiotherapy

Helping you access the right support.

Physiotherapy is only one part of helping someone remain safe, independent and well at home. With NHS and community rehabilitation experience, I understand how different health and social-care services can work together to support a patient.

I look beyond pain and movement to consider mobility, safety, equipment, care needs, the home environment and your ability to manage everyday activities.

It is regular part of my job to make referral and liaise with other health services such as District Nursing, Adult Social Care, Wheelchair services, Orthotics, care agencies, community rehabilitation teams, General Practices and many more. If I identify need of other services to be involved in your care I make referral and help you to get support you need.

YOUR FIRST APPOINTMENT

What happens when I visit?

The first appointment gives us time to understand the problem, assess what is affecting you and start a practical plan.

We talk

We'll discuss your symptoms, medical history, mobility and what you would like to achieve.

I assess you

This may include movement, strength, balance, walking, neurological, musculoskeletal or home environmental assessment depending on your problem.

We start treatment

Where appropriate, treatment and rehabilitation can begin during your first appointment.

You get a plan

You'll understand what I found, what we can work on and what you can do between appointments.
